Psyc 465 - Fall 2012 - Neuropsychological Assessment Report Assignment

Final Draft Due: No later than Monday December 10th at 11:59 pm (one minute before midnight) to receive full credit.

In this assignment you will write a sample NP assessment report using a variety of sources of information.

Before you start:

• You will need a computer that can play back either QuickTime or Flash video.
• You will need working audio : Speakers or Headphones (preferred).
• A word-processor that can save in one of these formats:
  • MS Word (.doc or .docx)
  • Apple Pages (.pages)
• A calculator for z-score formulas
• Graphing software. GraphPad Prism is available for Mac and Windows for free for CSUSM students. Click for GraphPad Prism site license

Materials:

• The assignment (MS Word Document) Download the assignment
• Reference articles for doing Raw, Scaled, T and Z-score conversions. Articles
• The patient video. Choose one of the two formats below:
  • QuickTime format (better for Macs or mobile devices)
  • Flash format (better for Windows PCs)

Turning it in:

All students must submit the assignment via the course website. Please follow these instructions carefully.

1. You will need your CSUSM login name (e.g. "jsmith001" if your name is John Smith) and your Student ID number (a 6 or 9-digit number)
2. Proofread your paper, and when you are satisfied, make sure you save it and then Quit the word processing program (If you don't save it and then quit the App, the last changes you made may not be uploaded).
3. Make sure the entire assignment is in a single file, and that you know which file is your final draft (check the time and date of the file so that you are uploading the right one).

5. When you have completed the upload, you will get a confirmation screen and a Print button. I suggest you print a copy for your records.
